Rx Clinical Services is an independent clinical pharmacy practice that is located in Eugene, Oregon. We partner with independent community clinics to enhance patient care. Our clinical pharmacists have completed advanced training in ambulatory care that enables them to provide the highest quality medical care through optimization of medication therapy. The medication recommendations, monitoring and treatments are centered upon evidence-based guidelines and established standards of care.

Our pharmacists build collaborative relationships with the prescribers and are the perfect complement to the entire medical team within the clinics in which we practice. This team approach improves the experience and health outcomes of our patients while boosting the efficiency and overall quality of care provided.

In-Clinic

Our clinical pharmacist(s) could come to your location and can provide the following services:

Annual Wellness Visits

Annual Wellness Visits are available at no cost to all individuals with Medicare Part B and is covered by Medicare every 12 months. This appointment will help to develop and update an individual’s personalized health plan. Our clinical pharmacists in cooperation with your primary care provider, will design a treatment plan to help prevent disease and disability based on your current health and risk factors. A list of services provided at these annual visits include:

  1. Review of current medications to ensure treatments are personalized based upon individual factors and specific disease states
  2. Obtain vitals as well as other routine measurements such as height and weight
  3. Lifestyle modifications including diet recommendations and optimization of exercise plan
  4. Screening for any cognitive impairment
  5. Personalized health advice
  6. Preventative measures to minimize future health risk and maximize health outcomes
  7. Recommendations and ordering of necessary labs and vaccinations | *Extra fees may apply for labs not related to screening.
  8. Advance care planning
AWV Add-On Services

AWV Add-On Services provided by our pharmacy team:

  • Intensive Behavior Therapy for Cardiovascular Disease: Cardiovascular disease includes all types of diseases that affect the heart and blood vessels. Intensive behavior therapy is designed for early prevention and risk reduction. During these face-to-face visits the clinical pharmacist ensures proper screening, medication recommendations (such as blood pressure management, cholesterol medications, aspirin use) and lifestyle modifications are discussed and implemented.
  • Intensive Behavioral Therapy for Obesity: Obesity places an individual at a greater risk for an extensive list of health-related problems. These problems include cardiovascular disease, high blood pressure, diabetes, liver disease and kidney disease. In collaboration with a patient’s primary care provider, our pharmacists will provide intensive behavior therapy for obesity that includes screening measurements, dietary assessment and behavior counseling to promote sustained weight loss through diet and exercise.
  • Alcohol Misuse Screening: Alcohol misuse screening is designed for early detection of habits that may potentially progress to alcohol dependency. If misuse is detected, resources for managing and overcoming these practices will be discussed and made available as deemed appropriate by our clinical pharmacists and your primary care provider.
  • Depression Screening: Medicare pays for 1 depression screening per year. Ensure your patients are being adequately screened by partnering with our pharmacists. If a patient screens positive, our team can have the patient follow up with their primary care provider and our pharmacist will be happy to recommend pharmacotherapy options and help monitor for safety and efficacy.
  • Smoking Cessation Counseling: Our clinical pharmacists are dedicated to creating a personalized plan to empower individuals in their smoking cessation process. Our approach includes a combination of lifestyle modifications, counseling and medication therapies that have proved to be successful.
  • STI Prevention Counseling: Sexually transmitted disease prevention and screening is an important healthcare service to help facilitate overall wellbeing and a healthy lifestyle. Safe sex practices and contraceptive options will be presented, enabling individuals to make personal choices that best fit their lifestyle and beliefs. STI screening or counseling will be provided to all eligible patients.
  • Diabetic Foot Exam: A complication of diabetes is peripheral neuropathy (nerve pain) that could potentially result in diabetic foot ulceration. A diabetic foot exam is an important tool to determine if diabetic neuropathy has developed. Early detection is vital so that proper interventions can be implemented for better diabetes control. Our clinical pharmacists will thoroughly discuss and explain the results of this exam and help develop a treatment plan to decrease the risk of further complications.
  • Cognitive Testing/Cognitive Care Planning: Cognitive testing will be provided for individuals with cognitive complaints or memory concerns. Proven assessment tools will be utilized to assess cognition function. Clinical judgment along with the results of these assessment tools will guide the necessary medication therapy that best fits each individual patient.
Chronic Care Management

Our clinical pharmacists have received extensive training and are uniquely skilled in the management of chronic conditions. In collaboration with your provider, they will customize a care plan to optimize your medication therapy. Chronic conditions that are eligible for coverage include Alzheimer’s disease, autism spectrum disorders, chronic obstructive pulmonary disease, diabetes, hyperlipidemia, hypertension, heart failure and osteoporosis. A comprehensive care plan will include clearly defined treatment goals and an overview of potential outcomes. This service will include the management of medications, lifestyle modifications, and details of social services or community services that are available to help maximize long term health outcomes and standard of living.

Intensive Behavioral Therapy
Transition of Care

The movement of a patient from one setting of healthcare to another is an important step in a patient’s treatment. This transition requires collaborative practice between healthcare providers to ensure that continuation of care is performed seamlessly to maximize patient’s care and health outcomes. Rx Clinical Services takes great pride in and is committed to ensure that patient’s transition of care is prioritized and patient’s experience within the healthcare system is a positive one.

Incident-to Evaluation and Management
Insulin Management

Rx Clinical Service is dedicated to a hands-on approach to the management of our patient’s insulin regime. We work collaboratively with our patients to recommend and ensure an insulin regime is created that best manages our patient’s blood sugars and fits their unique lifestyle. Our pharmacists take the time to educate our patient’s regarding diabetes, treatment options and lifestyle modifications to empower individuals to live confidently and correctly manage their diabetes. Our patients have continuous access to our pharmacists to help answer questions that arise while managing their blood sugar at home.

Inhaler Use & Instruction

Proper inhaler use is essential to the management and control of respiratory conditions such as asthma and COPD. Too often, complications surrounding the management of respiratory conditions is a consequence of improper inhaler technique and use. Our pharmacists invest the time to properly demonstrate, train and teach each patient on the proper use of their inhalers.

Diabetes Management & Education

Rx Clinical Services provides extensive diabetes education for patient’s living with and newly diagnosed with diabetes. Every patient will be provided with an in depth understanding of their condition and an optimized medication treatment plan that best fits their individual lifestyle. Our pharmacists are dedicated to a patient centric approach of diabetes management and will provide positive reinforcement and the medication knowledge that will enable patients to effectively manage their blood sugars at home.

The Medication Therapy Management program is designed to ensure your current medications regime is optimized. Our pharmacy team will perform a comprehensive review of your medication list to determine if any redundancy or any potential medication interactions exist. Our clinical pharmacists will take the time to listen and discuss how well your medications are working, any negative side effects, and whether your medication costs can be lowered.

Our Team:

Mary Onysko, PharmD, BCPS

Dr. Mary Onysko returned home to Lane County in May of 2018 to help create Rx Clinical Services and serve as our Director of Pharmacy. After graduating from Oregon State University with her PharmD in 2006, she completed a Primary Care Specialty Residency with Providence Medical Group in Portland, Oregon. After residency, she joined the faculty at the University of Wyoming School of Pharmacy and created an ambulatory care clinical practice site with the Swedish Family Medicine Residency affiliated with the University of Colorado School of Medicine in Denver, Colorado. She had been a faculty member with UW and Swedish for 11 years, and continues to provide contracted work for UW today.

Dr. Onysko is responsible for the creation and development of clinical pharmacy services for the company. She is also the point of contact and primary preceptor for students completing their Community, Ambulatory Care or Pharmacy Health Administration APPE rotations. Her primary clinical interests revolve around optimizing team-based patient care and revenue generation through billing for cognitive services. Personal goals include spending time with husband Randy and son Cameron, playing volleyball often, and getting to the mountains and coast as much as possible.

Nicholas Wood, PharmD

Dr. Nicholas Wood, an Idaho native, grew up spending most of his time playing sports and enjoying the great outdoors with his family. He continued to play baseball at Idaho State University where he would later earn earn his Doctor of Pharmacy Degree in 2017. After graduating with his PharmD, he completed a Ambulatory Care Residency with Family Medicine Residency of Idaho in Boise.

Some of his interests within family/ambulatory care medicine include nutrition/preventative medicine, diabetes management, mental health, and rural and emergency medicine. When not at work, Nick enjoys spending time with his wife, Megan, and his two children, Zuri and Tony. He is now thrilled to begin his career with Rx Clinical Services and River Road Medical Group in Eugene.

Dylan Ren

Dylan Ren is currently a third year pharmacy student at the University of Wyoming, School of Pharmacy. He was our first pharmacy intern. He attended the University of Wyoming pursuing an undergraduate degree in physiology before switching and enrolling in pharmacy school in 2016 after completing his prerequisite courses. Dylan will graduate in 2020 with a doctor of pharmacy degree, and a minor in honors.

Dylan’s primary responsibilities during his time at Rx Clinical Services included conducting patient visits, assessments, and collaborating with providers while developing and integrating pharmacists as part of the health care team. His topics of interest include diabetes, cardiovascular diseases, and medication safety. Upon graduation, he hopes to pursue residency training to further develop his clinical knowledge.

Jonathan Duke

Jonathan is a P4 student at Oregon State University/Oregon Health & Science University School of Pharmacy. Jonathan is also enrolled in the dual PharmD/MBA program through Oregon State University. He earned his bachelor’s degree in business administration and worked in the hospitality industry before returning to school to pursue his Pharm. D degree. His personal interests include fly fishing, drift boating, running and yoga.

He was Rx Clinical Services’ first P4 student to complete both his Ambulatory Care APPE and Business Administration APPE with us. He helped draft our initial Mission and Vision statements, streamline the Chronic Care Management (CCM) workflow, and provided direct patient care during Annual Wellness Visits (AWV) and Co-Consult Office visits.
